One registration form is required for EACH attendee. The registration deadline is May 1, 2008. (A $15.00 late fee per person will be charged for any registration postmarked after this date). There will be no exceptions relative to late fee charges.
Please type or print neatly (badges will be made from the information provided).
We can only accept church checks, cashier checks or money orders as payment for registration fees.We will not accept personal checks at any time or any church checks after registration deadline (no personal checks or church checks will be accepted at on-site registration, only cashiers checks, money order or cash will be accepted.)
